A medium base socket takes a light bulb you screw in and can swap. An integrated LED has the light source built into the fixture, so there is no bulb to change. Medium base gives you control over the light. Integrated LED gives you a slimmer fixture and nothing to replace until the fixture itself is done.
That is the short answer to medium base vs integrated LED. The longer answer is that neither one is better in every room. The right choice depends on how you want the fixture to look, how much control you want over the light, and what you expect to do on the day the light finally goes dim.
What is a medium base socket?
A medium base socket is the standard screw-in bulb holder used on 120-volt fixtures in North America. It is also called an E26 socket: the E stands for Edison screw, and the 26 is the diameter of the base in millimeters, which is roughly one inch. Any bulb sold with an E26 base fits it — an LED A19, a filament-style Edison bulb, a halogen, or an older incandescent.
Because the socket is a standard, the fixture and the light source are two separate purchases. You can put a warm 2700 K bulb in today and a brighter one in next year. You can raise or lower the output by changing the bulb rather than the fixture. And when the bulb finally fails, you replace a bulb, not the fixture on your wall. That is why medium base sockets are still the default on wall sconces and gooseneck lights.

What is an integrated LED fixture?
An integrated LED fixture has its LED light source built into the fixture body. There is no socket and no bulb. The LED boards, the driver that powers them and the housing that carries heat away are engineered as one unit, which is why these fixtures can be made far slimmer than anything that has to make room for a screw-in bulb.
Integrated LED is what makes a slim picture light or a low-profile flush mount possible. It also lets the maker control the beam, the color temperature and the color rendering precisely, because they are not designing around whatever bulb a customer happens to screw in. The trade-off is that the light source is not user-serviceable. When the LED eventually dims past the point of being useful, the fixture is the thing you replace.
On our side of the catalogue, direct-wire LED picture lights are the integrated products. A picture light has to be slim, has to throw an even wash down a canvas, and has to be gentle on the artwork — three jobs that are much easier to engineer when the LED is part of the fixture. Our own product copy puts it plainly: the LED picture lights are designed to be kind to your art by eliminating harmful UV and infrared light. A bulb-based fixture cannot promise that, because the maker does not control the bulb.

How long does an integrated LED last?
LEDs do not usually burn out the way an incandescent bulb does. ENERGY STAR describes what happens instead as lumen depreciation: the LED dims slowly, and its rated lifetime is set at the predicted point where light output has dropped by 30 percent. That figure is written as L70.
The ENERGY STAR Luminaires specification attaches numbers to it. To be certified, an indoor luminaire has to reach L70 at 25,000 hours or more and an outdoor luminaire at 35,000 hours or more. An inseparable luminaire — one whose LED cannot be removed, which is exactly what an integrated LED fixture is — has to reach 50,000 hours or more. The warranty minimums follow the same logic: two years for a residential luminaire, three years for an inseparable one. A medium base fixture has no such clock. The socket outlasts the bulbs, and you change bulbs when they dim.
Those hours are laboratory thresholds, not a promise about your hallway. ENERGY STAR is explicit that the higher the temperature at which LEDs are operated, the more quickly the light degrades and the shorter the useful life will be. An enclosed, poorly ventilated fixture will not reach the same number as a well heat-sunk one, which is one more reason how a fixture is actually built tells you more than the number on the box.
Can you replace an integrated LED?
Usually not, and it is worth being blunt about it. In most residential integrated LED fixtures the LED board and the driver are not sold as consumer spare parts, and the housing is not designed to be opened. If the light fails inside the warranty window, the route is a claim with the manufacturer. Outside it, replacing the fixture is normally the practical answer.
There are exceptions. Some manufacturers, mostly in commercial and architectural lighting, do sell replacement LED modules or drivers for their integrated fixtures. If long-term serviceability matters to you, ask before you buy rather than after. It is a fair question and a good supplier will answer it.
This is the honest cost of an integrated LED, and it is why the format attracts as much criticism as praise. What you gain in form, you give up in repairability.
Which is more energy efficient, medium base or integrated LED?
Both can be equally efficient, because in both cases the light is coming from LEDs. ENERGY STAR states that LED lighting products produce light up to 90% more efficiently than incandescent light bulbs, and that incandescent bulbs release 90 percent of their energy as heat. That gap is between LED and incandescent. It is not a gap between an integrated LED fixture and a medium base fixture with an LED bulb screwed into it.
Where an integrated design can pull ahead is heat and optics: the maker controls how heat leaves the LED and how the light is aimed, and both affect how much of the light produced actually lands where you want it. Where a medium base fixture pulls ahead is that you can fit a better bulb in five years, when better bulbs exist. Efficiency is not frozen at the moment of purchase.
What does each one actually cost you?
We are not going to put dollar figures on this, because they move with the fixture, the bulb and your electricity rate. The shape of the cost is what matters, and the two shapes are different.
A medium base fixture is usually the lower purchase price, followed by a small repeating cost: bulbs. Choose the bulb badly and you pay for it in light quality. Choose it well and you can improve the fixture later for the price of a bulb.
An integrated LED fixture concentrates the spend up front and removes the repeating cost. There is nothing to buy and nothing to change until the day the fixture is finished, and at that point the replacement cost is the whole fixture rather than a bulb.
Neither of these is automatically cheaper. They are two different distributions of the same money.
When a medium base socket is the better choice
• You want to be able to change the color temperature later.
• You want to choose the bulb style — a clear filament bulb reads very differently from a frosted A19 behind the same shade.
• You want a fixture you can keep for decades and service with an inexpensive part.
• You are matching an existing dimmer and want to pick a bulb that plays well with it.
• The fixture is a traditional or heritage style where a visible bulb is part of the look.
When an integrated LED is the better choice
• The fixture has to be slim — picture lights and low-profile flush mounts.
• The beam has to be controlled precisely, as it does over artwork.
• You want a sealed unit for a damp or exposed location.
• You genuinely never want to think about a bulb again.
• Color rendering and consistency matter more to you than future flexibility.
